DescriptionPOSITION SUMMARY: Primary functions are assessing, planning, delivering, coordinating, teaching and providing patient disease management (anticoagulation, hepatitis C, lipid, smoking cessation, vaccinations, etc.) care/treatment and department services. On occasions assumes charge responsibility. Serves as preceptor for new hires, students and resource person for other staff.RequirementsMINIMUM QUALIFICATIONS: EDUCATION: Graduate of an ACPE accredited School of Pharmacy.
