Key Functions and Responsibilities Works collaboratively with multi-disciplinary teams to optimize drug therapy. Provide direct patient care, including treatments assessment and medication management activities. Clinically evaluate all patients in a specified area daily. Ensure documentation is placed in the medical record regarding pharmacy recommendations, plans, intervention and medication reconciliation in medical records. Review patient’s drug regimen for an appropriate indication, dosing, allergy verification, potential drug interactions, lab monitoring, duplication of therapy, and medical documentation.
