Responsible for medication dispensation according to NYULMC policies and NYS Pharmacy Laws
Provides drug information and consultation to patients and health care providers: consults with the prescriber, the interdisciplinary care team, the patient and/or caregiver to identify/develop patient specific disease state therapy goals. Assure ongoing performance improvement of the pharmaceutical care services provided
Communicates effectively, and possesses strong verbal and written skills. Ability to articulate information in a clear and concise manner and to relate peers, prescribers, interdisciplinary care team, patients and/or caregiver
Utilizes Pharmacy Software (EPIC) to adjudicate claims: develops and designs clinical edits to adjudicate claims and enhance drug utilization
Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication
Conducts Comprehensive Medication Reviews, and Targeted Interventions to assess appropriateness of all clinical medications including dose and drug regimen to optimize medication therapy to improve outcomes. (Clinical appropriateness)
Review prior authorization requests and make determinations for drug products according to member benefits and coverage criteria
Assist with managing medication costs through coordination of benefits, assistance with prior authorization, formulary adherence, pill splitting, avoiding unnecessary and/or duplicate medication therapies and optimization of the medication regimen
Coordinates medication reconciliation across the continuum of care and monitoring to improve clinical outcomes and to help prevent unnecessary readmissions, lapses in care and adverse drug reactions. Attends interdisciplinary rounds to assist with medication-related plans for day of discharge.
Engages in active patient-centered medication therapy management through transitional care including medication reconciliation services, discharge assistance, and medication adherence/compliance calls: patient counseling on appropriate medical therapy, medication history, admissions and discharge medication reconciliations, facilitation of patient care. Identify additional therapies needed or therapies no longer needed assist with managing medication costs through coordination of benefits.
Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.) This process includes communicating with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
Provides vaccinations to patients as permitted by NYS Laws
Prepares compounded medications when required according to NYULMC policies
Maintains all Medicare part B claims data, provides support for all Medicare part B audits.
Prepare and Assist patients with medication adherence tools ( pill boxes)
Maintain pharmacy records according to NYULMC policies and NYS pharmacy law: Medicaid, Medicare Part D requirements and develop systematic reviews of prescription claims to meet strategic initiatives
Responsible for setting criteria for medication analysis, coordinate reports to clients, meet regulatory reporting requirements for drug utilization review and fraud and abuse reporting.
Provides clinical guidance and support to the assigned pharmacy technicians
