The Pharmacist is responsible for providing medication management, clinical support and consultative services for patients located in the Emergency Department, in addition to providing clinical and/or distributive support within the inpatient pharmacy.
The Pharmacist will work collaboratively with the health care team to improve drug therapy decision making and medication compliance. This position will also participate in education of pharmacy students and residents.
Shift will be 7 on, 7- off
Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
Provide clinical pharmacy services to all patients in assigned area. This includes dispensing of medication, completing review for all patients, monitoring, adjusting patient therapies and documenting changes according to department policies and pharmacy regulations.
Participate in emergent code situations: Code Blue, ED Red Team activations, Full Trauma activations, Code Malignant Hyperthermia and Rapid Sequence Intubations.
Interview patient/caregivers to obtain a medication history prior to admission.
Act as resource for pharmaceutical care to patients, partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives as appropriate.
Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences and actively participating in department clinical meetings and activities.
Participate in regulatory, quality assurance and process improvement activities as required.
Meets all regulatory requirements.
Uses available information technology and other resources to support day-to-day work operations

- Minnesota Pharmacist License
- Doctor of Pharmacy or Bachelors in Pharmacy degree
- Completion of a PGY1 residency with rotations in the emergency department/critical care areas OR 3 years hospital experience in critical care or clinical emergency department roles
- Completion of a PGY2 residency in emergency department or critical care area
